CPHC has been serving San Franciscans and the Chinese community since 1929. Since breaking ground in 1969 on the current and fifth site in Chinatown, CPHC has transformed itself from a traditional public health center into a full-service, community-oriented, patient-centered medical home that is part of San Francisco's safety net delivery system. Health Care for the Homeless Program "Alameda County Health Care for the Homeless Program (ACHCHP) is a federally-funded 330(h) Health Care for the Homeless program that has been providing health care and case management services to homeless persons throughout Alameda County since 1988.
